Carer Listening Event - Chesterfield

Carer Listening Event - Chesterfield

Event Date: January 17 2018

Event Location: Peak Edge Hotel, nr Chesterfield

Southern Derbyshire Clinical Commissioning Group (CCG) arranged a series of Carer Listening Events across Derbyshire and Derby City.  They have been allocated charitable funds to allow them to do this.

Three events have already been held this autumn but if you weren't able to make it there is still a chance to get involved. The 4th and final listening event is being held near Chesterfield in January.

The CCG want to use the event to talk to you as a carer openly, and to get feedback from you on a number of issues including:

  • Emergency Planning
  • Discharge Planning (this may change depending on availability)
  • Communications and the carers pathway
  • Assistive Technologies
  • Joined Up Care Derbyshire (Sustainability and Transformation Plan)

The CCG would also like to thank carers for the work you do and have arranged for lunch to be available.

Tuesday 17 January, 10am till 2pm, Peak Edge Hotel, Chesterfield, S45 0LW

The CCG hope that they have given enough notice for you to make suitable arrangements for the person you care for to be looked after whilst you are out.

Travel expenses will be available at 0.45p per mile if you are driving and bus/train fares as required.  In exceptional circumstances taxis will be refunded up to a maximum of £10 each way but this must be agreed in advance.

Numbers will be limited to 40 per event and the CCG ask that you attend only one event to ensure that there is space for everyone to have their input. The previous three events were well attended.

If you would like to attend, please contact Tina Brown, Commissioning Manager, Southern Derbyshire CCG by email: or tel: 01332 868756. Or you can print off this form and return it by post or email.
