Southern Derbyshire Clinical Commissioning Group (CCG) have arrange a series of Carers Listening Events across Derbyshire. They have allocated charitable funds to allow them to do this.
The CCG want to use the events to talk to you as a carer openly and to get feedback from you on a number of issues including:
The CCG would also like to thank carers for the work you do and have arranged for lunch to be available.
The various dates are given below; but if you live in the County area, but care for someone living in the City (say Alvaston), you would need to attend one of the City venues. Likewise if you live in Derby City, and care for someone who lives in (say) Belper, you would need to come to the event that takes place at one of the County venues listed below.
By doing this, the CCG can ensure that people get the right messages about the services. Whilst they and other services involved try to do things similarly across the city and county, there are likely to be some differences and it is important that you get the right information. If you are unsure about which date/venue to choose, please call or e-mail Tina Brown (details below).
The dates for DERBYSHIRE COUNTY are:
The dates for DERBY CITY are:
All the events start at 10am and finish at 2pm and the CCG hope that they have given enough notice for you to make suitable arrangements for the person you care for to be looked after whilst you are out.
Travel expenses will be available at 0.45p per mile if you are driving and bus/train fares as required. In exceptional circumstances taxis will be refunded up to a maximum of £10 each way but this must be agreed in advance.
Numbers will be limited to 40 per event and the CCG ask that you attend only one event to ensure that there is space for everyone to have their input.
If you would like to attend, please contact Tina Brown, Commissioning Manager, Southern Derbyshire CCG by email: email@example.com or tel: 01332 868756. Or you can print off this form and return it by post or email.September 17 2017
